Main City. This should be the main area that you want to advertise in. Likely the city your business is located in or the largest city in your service area (meaning the most difficult city to rank on Google for).
Service Area #1 and #2. These are the next largest cities that you service and want to optimize your site to rank for. You can leave one or both of these blank if you do not have any other service area.
State and State Abbreviation. Since you never know how someone might type the state in their search, it's a good idea to include the full state name and the state abbreviation.
Main Keyword. This should be your most important keyword and will be the one that appears toward the beginning of your title tag and description. This is the keyword you most want to rank for.
Similar Keyword #1 and #2. These keywords should be similar to the main keyword. If there are different variations to your main keyword that people might use to search for your service, add them here.
